Read the purpose of each form, complete as applicable and supply to the designated Award Leader or teacher in your school/parish. Your Award leader or teacher will be able to assist you with any questions you have.

Application & consent form for those wishing to start the Award

This form should be completed by each applicant wishing to start the Pope John Paul II Award. The completed application should be signed by the applicant and their parent/guardian. It should be returned with the enrolment fee to the designated Award Leader in your school/parish.

Parish & Social Activities record form for Award participants

This form needs to be completed by each applicant before starting the Pope John Paul II Award. The completed form should be returned to the designated Award Leader in your school/parish. This should be review regularly and updated if activities change.

Safeguarding Agreement form for organisations outside diocese and school

The Safeguarding Agreement form needs to be completed by any organistion helping participants of the Award where activities associated with the Award fall outside the scope of the diocesan/school and Diocesan Safeguarding Policy & Procedures. The completed form should be returned to the designated Award Leader in the school/parish.
